mothers2mothers (m2m) is an Africa-based NGO that unlocks the potential of women to eliminate paediatric AIDS and create healthy families across ten sub-Saharan African countries. m2m trains, employs, and helps empower women living with HIV to work as community healthcare workers in understaffed health centres and underserved communities. Through a peer-to-peer approach, these ‘Mentor Mothers’ deliver a range of health services, advice, and support to women and their families. Started in Cape Town, South Africa in 2001 with an initial focus on preventing mother-to-child transmission of HIV, m2m has innovated and built on its strengths and now provides family-centred support for a range of related health and social issues spanning pregnancy, birth, childhood, and adolescence. It also partners with governments and other NGOs on the adoption of its programme to spread the Mentor Mother Model and its benefits. m2m has a track record of scaling its services and proven impact. Job Description

1.    Ensure that the employees with identified health condition/s, conditions such as hypertension and diabetes, are appropriately and regularly assessed and referred for treatment. 
•    Provide the baseline assessment and categorise into high and low risk
•    Link appropriately to the Medical Officer/facility for more assessment
•    Schedule the revisit as per need for review
2.    Advise the employee to adhere to an individualised treatment plan and answer any questions they may have.
•    Conduct telephonic follow up for the staff based on the initial guidance
•    Provide continuous support as needed
•    Monitor staff for adherence to treatment as well as scheduled
•    Develop plan on the implementation of support to the high risk staff
3.    Maintain ongoing consultations with the employee to manage medical condition/s.
Provide wellness support to staff with certain risk factors (e.g., obesity, hypertension, Asthma diabetes), as well as wellness checks quarterly thereafter. 
Refer staff members to appropriate treatment as required.

4.    Document lessons learned to inform roll-out in other m2m countries.
Hold monthly calls with County Lead, HR representative and CMU Lead to monitor progress and to provide assistance and guidance, as required.  

5.    Reporting
•    Wellness assessment conducted as well as risk level assessed (using m2m’s risk framework) conducted, and documented for all identified country staff
•    Wellness support provided to all staff with those who have certain risk factors (e.g., obesity, hypertension, and diabetes) prioritized for assessment, followed by wellness checks quarterly thereafter.
•    Monthly wellness calls made to identified staff to follow-up on their initial assessment, document progress/adherence to treatment, and discuss any challenges.
•    At end of pilot, report submitted to County Lead and HR representative, determine frequency, includes but is not limited to:
                    Number of assessments conducted 
                    Number of staff with unmanaged medical conditions such as hypertension or diabetes

Job Requirements

Qualifications and Experience:
•    Diploma in General Nursing
•    Diploma in Midwifery, Primary Health Care or related Courses
•    Registration with Lesotho nursing Council as a Nurse Midwife
•    Post graduate qualification in HIV/AIDS with expertise in HIV care and treatment would be an added advantage
•    Prior clinical work experience in providing Primary Health or Nurse Clinician will be an added advantage
